Topical Exosomes in Skincare: What They Are, Why They're Everywhere, and Are They Actually Safe?

But "exosome" doesn't mean one thing, and the difference between what's in a bottle and what happens in a clinic is the part most articles skip over.

The Distinction That Actually Matters: Topical vs. Injectable

No exosome product (topical or injectable) has been approved by the FDA for a therapeutic or drug claim. But that single fact covers two very different categories of risk:

  • Injectable exosome therapy, typically offered in medical spas or clinics as an add-on to microneedling, laser resurfacing, or as a standalone injection, uses products that the FDA classifies as unapproved biologics. As of 2026, the agency has issued warning letters to multiple exosome manufacturers for marketing unapproved new drugs and unlicensed biological products, and a 2025 case series published in the Journal of Cosmetic Dermatology documented severe cutaneous reactions, including granulomas, following unregulated intradermal exosome injections.
  • Topical, cosmetic exosome ingredients - the kind found in leave-on serums and moisturizers - sit in a completely different regulatory category. Cosmetic ingredients don't require FDA pre-market approval the way drugs and biologics do, provided the brand doesn't make disease-treatment claims. As one dermatology-focused review put it, nothing about topical, plant-derived exosome-like cosmetic ingredients should be confused with unapproved injectable exosome treatments - they occupy a completely different risk category.

Dermatologists interviewed by NBC News make the same distinction from the clinical side:

most over-the-counter exosome serums use lab-made or plant-derived exosomes, which are considered safe and hydrating, while human-derived exosomes with more regenerative claims are typically reserved for in-office use and carry a much higher price point.

In short: what's inside a well-formulated topical serum, and what's happening when a provider injects an unapproved biologic into your dermis, are not the same conversation.

Where PDRN Fits In

You'll often see exosomes formulated alongside another trending K-beauty ingredient: PDRN (polydeoxyribonucleotide), sometimes nicknamed "salmon DNA." PDRN is a purified nucleotide fragment, originally researched for wound healing, that works by activating adenosine A2A receptors in skin cells. The receptors were shown to stimulate fibroblast metabolic activity, support wound healing, and reduce inflammation while also triggering VEGF production in poorly perfused tissue. A 2025 literature review in the Journal of Medical Informatics Technology found that PDRN activates those A2A receptors to stimulate angiogenesis, cell proliferation, and reduced inflammation, and that pairing it with vitamin C and niacinamide further supports the skin's antioxidant defenses and extracellular matrix.

As with exosomes, PDRN's most dramatic clinical results come from injectable "skin-booster" treatments, but non-injectable, topical PDRN delivery is a legitimate and rapidly growing category in its own right.
